ConnectionHealth is a 501c3 nonprofit organization that was founded in Birmingham, Alabama. ConnectionHealth is dedicated to providing community based support to our clients so they can live healthier lives. We currently provide services to residents of Jefferson County and throughout the Black Belt region of Alabama.

Building a Team of Community Health Workers
|
Community Health Workers work in partnership with a patient's care team to provide a variety of services through education, advocacy and social support. They may support access to primary or preventive care by connecting patients to medical homes, teaching concepts of prevention, assisting with chronic disease management, encouraging healthy lifestyle choices, helping keep medical appointments, increasing adherence to treatment protocols and connecting patients to resources in the broader community.
ConnectionHealth is the only nonprofit organization in Alabama that trains and employs community health workers to address the needs of Alabama's most vulnerable citizens. |
