Alabama CEAL

Alabama CEAL is a National Institutes of Health grant designed to address COVID-19 throughout the state of Alabama. ConnectionHealth provides Community Health Worker (CHW) services in the proposed service areas of Jefferson, Mobile, and Dallas counties. CHWs are currently participating in outreach activities across the state. CHWs have a list of community events and are also welcome to add events where they can disseminate materials.

Preventing Diabetes in the Deep South (PreventT2)

Preventing Diabetes in the Deep SouthPreventing Diabetes in the Deep South: Extending Partnerships and Adapting Interventions to Reach Rural Communities at High Risk. The objective of this 1-year project is to conduct a pilot study to assess the feasibility of an adapted Diabetes Prevention Program (DPP) intervention (telehealth + CHW) with and without metformin for diabetes prevention in the Alabama Black Belt.

Sub-award No. 102351-0001-CH-01 for the “Supporting Community-Based Organizations to Increase Vaccination Coverage across Different Racial and Ethnic Adult Populations Currently Experiencing Disparities” project.

Alabama United (UAB COMET)

Alabama United was a project developed in response to a National Institutes of Health emergency funding opportunity focused on community-engaged research RE: COVID-19 testing among vulnerable populations. The proposed program aimed to employ implementation strategies to improve COVID-19 testing in rural communities across Alabama. ConnectionHealth was a partner in CHW recruitment, training, and program implementation. Recently, the Scope of Work has pivoted to include survey administration in designated geographical regions.
